Is it mandatory to fill in a tax declaration form with Quellensteuer and under 120k?

I keep getting these forms from the tax office, but they seem to be quite burdensome to decrypt so I have put them aside.

What I'd like to know is that if I live in canton Zurich and earn less than 120k and pay Quellensteuer, do I need to fill in the form regardless? The forum says NO in the older posts, but some of my colleagues say YES.
